Welfare Benefit Plan Form 5500 Services
Plan sponsors establish employee welfare benefit plans, such as medical, dental, vision, and group term life insurance, to attract, retain and reward employees.
However, administering and preparing the required Form 5500 return (Annual Return/Report of Employee Benefit Plan) requires an understanding of a complex set of laws and government regulations. We provide value-added services to help plan sponsors and administrators with our Form 5500 preparation services.

- Knowledgeable and Experienced Staff - You will benefit from the experience of our well-versed group of professionals who have administrative and practical experience and will respond to your questions.
- Quality - We established internal review processes and procedures to ensure quality service. We will prepare the return and file it electronically with the Department of Labor after it is signed by the plan sponsor/administrator.
- Simplified Questionnaire - You will receive a welfare plan questionnaire that includes information from the prior year’s Form 5500. All you need to do is review it for any changes that were made to the plan since the prior year and supply us with the requested information for the current year.
- Insurance Brokers - We work with various insurance brokers to simplify the process of collecting required plan insurance information.
What is a Form 5500?
A Form 5500 is an informational return required by the Department of Labor and Internal Revenue Service that identifies basic plan information (plan sponsor, administrator, type of plan and benefits, number of participants), financial statement information (if a trust is used), and insurance information.
The plan sponsor and plan administrator of a welfare benefit plan must electronically file a Form 5500 for a plan within 7 months after plan year end unless a 2 1/2 month extension of time to file the return is requested.
Who Must File a Form 5500?
Generally plan sponsors that provide health, dental, group life, disability or other employee welfare benefit plans must file a Form 5500 if:
The plan is unfunded and/or fully insured and has 100 or more participants as of the beginning of the plan year,
OR
Benefits are paid from a trust or plan account and not through insurance or the general assets of the employer.

For each employee welfare benefit plan, we will:
- Prepare a Form 5500 and all related schedules that can be filed electronically with the Department of Labor
- Prepare a summary annual report for you to disseminate to eligible participants and beneficiaries
- Assign a specialist and a manager to each client’s plan
- Review the required information supplied to us
- Assist clients and insurance brokers with their inquiries
